Your first visit to Advanced Dermatology Mohs & Laser Surgery Center involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. What to Bring to your Visit

  • Government Issued Photo ID
  • Insurance Card (Please be prepared to present this card at every visit)

  • Copays
  • Referral (if applicable)
    • Be advised that it is your responsibility to determine your need for a referral.  If one is required, and you do not obtain one prior to your visit, you will assume payment responsibility.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this Web site.

Be advised that we do have allow patients to be up to 15 minutes late for their appointments.  If you arrive more than 15 minutes late, we will ask you to reschedule.  We also have a 24-hour cancellation policy.  If you miss your appointment, without giving us 24 hours notice, you will receive a bill for that missed appointment.
